1. Correctly identify your assigned social determinant. Briefly define the determinant and describe how it impacts healthcare outcomes.
The ability for individuals to seek and obtain medical care when they need it is of profound importance to their health and their community. Access to health care can be limited for a variety of reasons. Socio-economic factors can result in health disparities and access to health care. Healthy People 2030 initiatives notes that, social determinants of health, of which health care access and quality, is one of the 5 components, can impact the quality of an individual’s well-being (health.gov,2022). According to Nuako et al (2022), access to healthcare was limited for individuals who live in rural areas, who have fewer numbers of providers in the area and for those who have limited access to transportation. Other reasons such as inability to afford medications and lack of insurance, were of economic nature according to Nuako et al, 2022. Another reason why patients were unable to have access to a provider was because of limited number of providers in the areas, especially in rural ones. This resulted in delayed scheduling of patients for appointments. Nuako et al (2022), notes that this could have resulted in contributing to diagnoses that could have been prevented.
2.Select an objective related to your assigned social determinant from the Healthy People website linked above. Identify the status of the objective and the population to whom the objective applies
Healthy People Objective: Increase the proportion of adults who get recommended evidence-based preventive healthcare- AHS-08
This objective is aimed at adults. According to Healthy People, this objective has not shown any improvement since it was initiated (Health.gov,2022).
3.Provide at least three appropriate questions related to the chosen objective that the NP could incorporate into the health history and interview to assess the client and family?
To provide evidence-based preventive healthcare to adults as recommended by Healthy people Initiative, the NP can incorporate the following 3 questions into the health history and interview that will aid in assessing the client:
1. What is the specific clinical need of this client?
2. Where can the highest quality of answers come from?
3. Is the information obtained sufficient?

4.Identify at least two online, national, or local resources and explain how the resources could help meet the chosen objective for at risk clients or families
Clinical Practice Guidelines
CDC.gov
Clinical Practice Guidelines contain recommendations on how to treat or manage a patient diagnosis. The recommendations are made by experts in that field and are usually evidenced-based knowledge. The CDC website is a government provided resource that has up to date information on the disease, prevention, management and other resources that can aid providers in making the best decisions regarding patient care.

My social determinant is social and community context. Social and community context relates to the relationships people have with they’re environment and the people within it and how it impacts health. For example, an adolescent who is raised in a household in which their parents are unsupportive leads to disparities in mental and social health.
Select an objective related to your assigned social determinant from the Healthy People website linked above. Identify the status of the objective and the population to whom the objective applies.
An objective related to my social determinant is: increase the proportion of children whose family read to them at least 4 times a week (Office of Disease Prevention and Health Promotion, n.d.). The status of this objective is getting worse. The population that it applies to is children the age of 5 and under.
Three questions the NP could incorporate into the interview:
1. Is your child able to recognize and name pictures from a picture book? How many?
2. Do you read to you child? Are you able to make time to read to your child?
3. Are you able to afford books so you can read to your child?
Resources Available to support objective:
1. The Book Fairy Pantry Project offers free used children’s books to families in need. Providing reading materials for children helps combat the potential reason that children are not read to in the event that the families are unable to afford reading materials for their children (Book Fairy Pantry Project, n.d.).
2. Reading is Fundamental is an organization that provides resources to families such as reading activities, literacy activities, and how to help children read. This would help meet objective by not only providing guides for how to incorporate reading into the child’s life but also connects families with education on the importance of reading to children and supporting their future literacy (Reading is Fundamental, n.d.). An article by Ece Demir-Lira et. al found that early reading resulted in later reading comprehension skill (Ece Demir-Lira et. al, 2019).
